So yeah, stop getting shot or jumping off cliffs and you should be fine. Also, unless you're on survival mode, they will also heal over time. Broken limbs can be repaired using stimpacks or by visiting a doctor and getting healed. When those limbs lose all their health (due to being shot or you taking damage by falling or something), they become crippled, causing debuffs.
